KP Population Health Management Initiative (PHMI)

Funded by Kaiser Permanente with support from the California Department of Health Care Services, JSI, with four nationally renowned transformation partners, leads technical assistance for the Population Health Management Initiative (PHMI).

PHMI supports community health center (CHC) grantees across multiple regions of California in implementing strategies to increase access, address disparities, and improve outcomes for over one million patients—specifically focusing on children, pregnant people, and adults with chronic conditions, behavioral health conditions, or preventive health needs. Through an Action Network model, participating  CHCs receive one-on-one coaching and subject matter expertise, HIT support, and engage in peer networks to advance their goals.

The initiative aims to improve clinical outcomes and sustain population health management strategies, ultimately reinforcing California’s Medi-Cal reform efforts known as CalAIM.
